 Notice of Privacy Practices


This notice describes how medical information about you may be used and disclosed and how you can get access to this information. 
 
We understand that your health information is personal to you and we are committed to protecting the information about you.  This Notice of Privacy Practices describes how the Visiting Nurse Services will use and disclose protected information and data that we receive or create related to your health care.

Patient Rights and Responsibilities

Each patient/client of the Visiting Nurse Services of Newport and Bristol Counties has the right to be informed of his or her rights in a manner that they understand. 

This notification is given in writing in advance of furnishing care or during the initial evaluation visit before the initiation of treatment.  A copy of the written notice of rights signed by the patient/client will be maintained in the patient/client’s home and at the VNS in the patient/client's medical record.  It is the duty of this agency to protect and promote the rights of patients.

Comments, Questions and Complaints
If you would like to submit a comment or if you feel your privacy rights have been violated, you may file a complaint with our office or with the Secretary of the Department of Health and Human Services.  You may file a complaint with our office by sending a letter outlining your concerns to the person listed below.  All complaints will be investigated thoroughly and you will not be penalized for filing a complaint.  Please file a complaint with:
